Directing My Life Transition Center

The Directing My Life Transition Center is a community-based program designed for young adults with disabilities who are ready to take the next step toward greater independence.

Unlike a traditional classroom, our participants spend each day practicing real-life skills in a supportive, encouraging environment. Through hands-on learning, work experiences, community involvement, and individualized coaching, participants build the confidence and abilities they need for adulthood.

Our program focuses on:

  • Independent living skills

  • Employment readiness and work experience

  • Social and relationship skills

  • Communication and self-advocacy

  • Executive functioning and organization

  • Health, wellness, and daily routines

  • Community safety and navigation

  • Personal responsibility and confidence

Every participant follows an individualized roadmap based on their goals, strengths, and support needs. Whether the goal is employment, greater independence at home, college success, or becoming more involved in the community, we provide practical experiences that prepare participants for life beyond high school.

Our first cohort is designed for individuals with lower support needs who can participate in small-group instruction and community-based learning with minimal assistance. Enrollment is intentionally limited to ensure personalized support and meaningful opportunities for growth.
